Although clinical and public attention is often focused on the categorically-derived DSM-V diagnosis of Narcissistic Personality Disorder\, narcissistic wounds stemming from attachment failures are a more frequent occurrence in the development of the self. These arrests in the full formation of the self and their subsequent re-appearance in later relationship interactions are often referenced in a wide range of treatments.  \nIn this workshop\, we will consider how Kohut’s self psychology and current innovations in attachment theory can help us understand narcissistic wounds and treatment issues across the dimension of narcissism. Further\, we will focus on the role that hypnosis can play in a relationally attuned therapeutic relationship towards facilitating attachment repair in patients with developmental arrests associated with narcissistic wounds (e.g. false-self identity formation\, affect dysregulation\, boundary difficulties\, etc.). Yet\, its underlying neural mechanisms have only recently begun to be elucidated through neuroimaging and electrophysiological studies. This presentation will summarize current evidence on how hypnosis modulates brain activity and connectivity across key networks involved in consciousness\, attention\, and pain processing. Findings show that hypnosis alters the balance between internal and external awareness networks\, disrupts activity within the Default Mode Network\, and engages regions such as the anterior cingulate and insular cortices that mediate emotional and sensory components of pain. Differences between highly and low hypnotizable individuals further highlight distinct attentional and inhibitory mechanisms that may influence clinical responsiveness. We will also review neurophysiological data supporting the use of hypnosis in acute and chronic pain\, oncology care\, phobias\, and functional neurological disorders. Finally\, emerging approaches combining hypnosis with virtual reality will be discussed as promising tools to enhance patient comfort and engagement in clinical settings. The nature of ongoing symptoms and distress often result in a ‘negative set’ that of itself impairs the ability to fully engage in therapy and limits hope for lasting improvement. We will explore the model of pain\, discuss the nervous system’s response to ongoing distress\, and offer a model for hypnotic suggestion to strategically alter perception and meaning-making involved in this process. Specific methodologies to ‘deconstruct’ the negative mental set that commonly accompanies chronic and intractable symptoms such as pain\, anxiety\, and depression will be discussed\, including specific hypnotic approaches to challenge negative self-talk\, to interrupt negative loops in thinking or feeling states\, and to offer greater distance from or qualitative awareness of such perception. This new theory is a dual-process model that provides a comprehensive understanding of the rational system and the experiential system. While these two systems work together in synergy\, they function quite differently in terms of their characteristics and methods. The rational system is influenced by logic and reason\, demands significant cognitive resources\, and operates with concentrated effort. On the other hand\, the experiential system is powered by emotions\, relies on associations\, and encodes reality through images and feelings effortlessly. According to the adaptive experiential theory\, the ability to engage in complex hypnotic responses can be attributed to an individual’s capacity to adaptively shift from processing predominantly via rational system to the experiential system. Having a stronger association with the experiential system leads to changes in how reality is processed\, enabling hypnotic suggestions to be internalized\, reified\, and put into action with less interference from the rational system. This presentation will focus on the implications this theory has for the clinical use of hypnosis emphasizing the idea that content and language of hypnotic suggestions matter.  \nLearning Objectives:  \n\n Identify and describe the rational system and the experiential system and describe at least three core differences between the two systems.\n2.
